A unique and innovative model
The Aline-Chrétien Health Hub - what's that?
The Aline-Chrétien Health Hub is a unique and innovative model that will bring a wide range of specialized and community healthcare services under one roof. Here, residents of the greater Orléans area, from newborns to seniors, have access to integrated programs close to home, focused on their needs and offered in both official languages.
The development of the Aline-Chrétien Health Hub project is the result of a collaborative effort among the following service providers:
- Bruyère
- CHEO
- Eastern Ottawa Resource Centre
- Geriatric Psychiatry Community Services of Ottawa
- Home and Community Care Support Services Champlain
- Hôpital Montfort
- Ottawa Public Health
- ParaMed
- Youth Services Bureau of Ottawa
The concept of the Health Hub will enable these partners to work together in order to offer complementary care. For the client, this improves the coordination of services, provides easier access to care and minimizes travelling.
It is a priority project for the Ontario government, which sees it as a promising model for the evolution of Ontario's health care system and will reduce the need for hallway medicine.
