Patient Portal

Various information about your appointment is available via the Patient Portal.

Access to your medical imaging online with PocketHealth

Patients at Montfort can now easily access their medical imaging with PocketHealth. With just a few clicks, you can use this secure online platform to view, store and seamlessly share your images anytime, on any device. Fees may apply.

How to access your records

1.    Go to pockethealth.com/montfort to request your images.
2.    Once your images are ready to view, you’ll receive an email from PocketHealth with a secure link. 
3.    Click on the link received by email to download and share your images. 
4.    Physicians receiving shared images can view or import these images, with no software or account set up needed.

Request for access to records

If you have a request for access to general records, a request for access to personal information, a request for correction to personal information or a request for access to personal health information (clinical records; release of personal health information; confirmation of birth, hospital stay or death), you can submit your request online or contact the Access to Information and Privacy Protection Office. 

This applies if the request is for you, a loved one or a patient (for a lawyer’s office, doctor’s office or other).

Bio-Test Laboratory Results

Patients of the Bio-Test Laboratory are able to access their own results on-line, via a secured patient portal for patrons of the Aline-Chrétien Health Hub.

To modify, confirm, book or cancel an appointment 

To send a medical referral to a clinic or department or if you have to reschedule or cancel an appointment, please contact the clinic or department directly. For confidentiality reasons, only the specific clinics can cancel, change or modify an appointment.

You will find a complete list of clinics and services under the Programs and Services tab in the main menu. Then click on the clinic or service you wish to contact; contact details are provided on each clinic or service page.

To request an appointment for any of the medical imaging services (X-ray, ultrasound, mammogram, MRI and bone densitometry), please email rendezvousimagerie@montfort.on.ca. Be sure to include your medical referral as a PDF attachment (photos of referrals are not accepted) and specify that you would like an appointment at the Aline-Chrétien Health Centre.

Services at the Aline-Chrétien Health Hub are offered by eight different partners, each with different medical or self-referral processes. 

To find out how to refer a patient to a specific Hub service, hover your mouse over the "Programs and Services" menu option on our website and select the program of your choice. The contact or referral information for that particular program will be detailed on each specific page.
