Based on a interpersonal psychotherapy (IPT) model, the program's goal aim to help Francophones aged 16 to 24 make the transition from youth mental health services to adult services.
The content of the group is tailored to the diagnoses, but more importantly to the goals of the participants. We also offer a referral service throughout the program, to help facilitate treatment for each participant's needs.
Our closed group meets on a weekly basis for 12 weeks.
The CAJAR team consists of a psychiatrist, a social worker and a psychotherapist. To allow someone to access this group, a health care professional must complete a referral form. The request will be assessed by our central intake staff.
Prior to starting the group, a participant should expect to meet with the psychotherapist or social worker assigned to the group for a minimum of two individual sessions to complete the assessment and orientation to the group.
This program is a partnership between Montfort and the Young Adult Mental Health Transition Service of CHEO.
To be eligible to receive this service, you must:
- have the following form completed by a family physician or a nurse practitioner (who accepts return of care)
- be a resident of the area serviced by Home and Community Care Support Services Champlain
- have a valid Ontario Health card (OHIP)
Exclusion criteria:
- Eating disorders as a primary diagnosis
- Exacerbation of a substance use disorder
- Intellectual disability or borderline intelligence
- Active psychotic symptoms
- Current hypomanic or manic episode
To refer a patient to this service, a family doctor or nurse practitioner must fill out the referral form, accept return of care and send the completed form by fax to 613-748-4938.

This service at the Aline-Chrétien Health Hub is offered by Hôpital Montfort.
