Care coordinators will be on-site at the Orléans Health Hub to assess clients to better understand your needs and refer you to the program or service that may be right for you in the community.
The Care coordinators are regulated health professionals, with expertise in nursing, social work, occupational therapy, physiotherapy, dietetics or speech therapy. Our staff may ask some questions to better understand your needs.
Anyone can make a referral to the Care Coordinators: a family doctor, friend, family member, or yourself.
Care coordinators can help clients get access to the following services: nursing (doctor referral required), personal support, physiotherapy, occupation therapy, nutritional counseling, speech therapy, social work, respite care, long termcare placement and school health support services.

If your health-care provider has already made the referral for you, you can expect a phone call from us once the referral has been processed.
