The Montfort Neonatal Follow-Up clinic is a developmental screening clinic for infants at risk of developmental delay. We work in collaboration with family physicians and pediatricians. We aim to provide guidance and support for children and families. This clinic addresses the needs of infants and children who have, or may develop a physical, developmental and/or behavioural disability.
Our focus is on identifying developmental concerns as early as possible to introduce early intervention.
Visits to this clinic usually happen at 4, 8, 12, 18, 24, 36 months (corrected for the prematurity) and at 4 years old.
In addition to developmental screening, our clinic plays an important role in the education and support of pediatricians and family medicine physicians who follow high risk and ex-premature infants.
Every child has their own unique needs, and our team is dedicated to addressing them. We are happy to answer any questions or concerns you may have regarding your child's development. Our clinic responds to the Provincial Council for Maternal & Newborn Health (PCMCH) mandate within the Ottawa and Champlain region.
The clinic’s team follows children with medical conditions that increase risks for developmental problems. These are example of medical conditions that would lead to a referral for follow-up:
- Gestation: 29+0 to 32+0 weeks
- Birth weight: less than 3rd percentile or head circumference <3rd percentile
- Symptomatic or recurrent hypoglycemia over 6 hours requiring intensive care monitoring and treatment (ex. Glucagon, diazoxide)
- Neonatal abstinence syndrome (NAS) requiring pharmacological treatment
- Multiple pregnancy > = 3 and > = 30 weeks gestation
- Twin to twin transfusion syndrome born between 30 to 37 weeks gestation
- Periventricular leukomalacia 31 weeks gestation and older up to term
- Mild neonatal encephalopathy or moderate that did not require level III NICU
- At the physician's request
We also accept infants who were born outside of Ottawa, particularly those living the East end of the Champlain region and who fall under the above clinic admission criteria.
You must be referred by a doctor, pediatrician or healthcare professional to use the services of this clinic.
To refer a patient, send their consultation request and documents to the attention of the Neonatal Follow-Up Clinic.T he discharge summary of the NICU stay should be sent with the consultation request.

What to bring
- Your child's health card
- A list of the developmental resources that your child has been referred to or is receiving (e.g. infant/child development, physiotherapy, occupational therapy, speech-language pathology, etc.)
- A list of medication and vitamins that your child is taking
- A completed ages & stages questionnaire that you will receive by email from our clinic.
Visit length
The visit should last 90 minutes, which includes the assessment by the nurse and the physician. We will send a report of the visit to your family doctor. Remember that appointments in this clinic do not replace regular visits with your child's doctor.
